New Patients

We look forward to meeting you and your family. We have a very friendly, compassionate, and efficient staff that will guide you from start to finish.

To ensure that things run smoothly for you first appointment, we ask the following:

You will receive registration paperwork in the mail when your appointment is made. Please take the time to fill this out prior to your appointment. If you misplace your forms, you can download them here:

On the day of your appointment:

  1. 1. Please arrive at least 15 minutes early
  2. 2. Bring all medications or a list with you to all appointments
  3. 3. Please bring ALL of your insurance cards
  4. 4. Please be prepared to pay your co-payment, deductible or co-insurance at the time of the appointment.

During your initial visit with us, lab work and notes from your referring physician will be reviewed. Kidney Disease is monitored through regular lab work. You will have plenty of time to ask questions or discuss concerns that you may have.

Insurances

We accept most insurances. We participate with Medicare, all Anthem Products, Optima Health, US Healthcare, Medicaid, Southern Health, Carenet, Humana. Patients will be required to pay deductibles / co-payments at the time of service. We file all claims as a courtesy to our patients. However, the patients are ultimately responsible for their accounts with our office. Patients who do not have insurance have the option of enrolling in a payment plan prior to service.
